YWCA is looking for a full time case manager for our Transitional Housing program. The right candidate will have the opportunity to advance to Associate Director for the program. Candidate should have case management experience and background but also have the ability to participate in grant writing, meetings with funders, and ability to manage time and resources effectively. Budget experience is a plus. Candidate should share the philosophy of “Teach a Man to fish” rather than “give a man a fish”. Our program is focused not just on the immediate emergency need of the family but on the long term goal of creating self-sufficiency, responsibility and personal accountability.

JOB TITLE: Case Manager, Transitional Housing Program
REPORTS TO: CEO
POSITION SUMMARY:
Housing case managers assist clients in obtaining housing and by providing services and referrals for a variety of needs including general assistance, job readiness, substance abuse counseling/programs, mental health, medical care, etc.
QUALIFICATIONS:
- Bachelor’s degree in social work, human services or related field and 1 year work experience working with victims of domestic violence or homeless population
- Experience in providing case management services to diverse populations
- Excellent interpersonal and written communication skills
- Compassionate concern for those who are homeless
- Experience with computers and proficient typing skills
- Ability to complete necessary paperwork, and maintain client files
WORKING CONDITIONS:
- 40 Hours per week – occasional nights and weekend depending on client needs.
Time will be spent:
- Conducting home visits to clients at scattered site apartments
- In the office completing paperwork and maintaining client files
JOB RESPONSIBILITIES & PERFORMANCE STANDARDS:
- Assist clients in locating, securing, furnishing, moving into, and maintaining appropriate housing
- Make referrals for general assistance, benefits, medical care, mental health, community agencies/eligible services, substance abuse counseling/programs, etc.
- Accompany clients to appointments, if necessary
- Train individuals in job readiness and assist in job searches
- Educate & assist clients in daily living skills
- Assist clients with supportive services and/or other types of assistance as needed
- Maintain contact with clients
- Maintain up to date client files
- Complete progress notes for all client contacts
- Develops Independent Development Plans with each client around needs
- Enters client data into BAS-NET
- Attend and participate in monthly meetings
- Be knowledgeable regarding issues affecting our target population
- Be knowledgeable regarding community/state resources
- Other duties as needed/assigned
